HIPAA Notice of Privacy Practices

Michigan Service Animals and its Michigan-licensed clinicians are covered healthcare entities subject to the Health Insurance Portability and Accountability Act (HIPAA). This notice describes how your mental health information may be used and disclosed.

What Information Is Protected

Protected Health Information (PHI)

Your mental health assessment information, clinical notes, diagnosis impressions, and any health information you share during your telehealth evaluation with a Michigan-licensed clinician is PHI protected under HIPAA.

Electronic PHI (ePHI)

Electronic records of your evaluation are encrypted and stored securely. Michigan Service Animals uses HIPAA-compliant telehealth platforms and secure data storage for all client electronic health information.

Minimum Necessary Standard

Michigan Service Animals and its clinicians use and disclose only the minimum necessary information for each permitted purpose. Your diagnosis is never shared with landlords — only the ESA or PSD recommendation.

Permitted Uses and Disclosures

Treatment

Your information may be used by your Michigan-licensed clinician to provide evaluation services. Information may be shared with other treating providers with your authorization.

Healthcare Operations

Michigan Service Animals may use de-identified information for quality improvement, training, and compliance purposes. No identifiable client information is shared for commercial purposes.

Required by Law

We may disclose your information when required by Michigan law or federal law — including mandatory reporting requirements and legal proceedings as required by applicable law.

With Your Authorization

We will not disclose your PHI for any other purpose without your explicit written authorization. You may revoke authorization at any time in writing.
